Submit Medical History Form
Your Patient Advocate will send you our Medical History Form. Once submitted, your Patient Advocate will call you to confirm we have received your form. Determining your eligibility for stem cell therapy typically takes 24-48 hours.
Schedule your e-visit
Once a decision is made, your Patient Advocate will contact you and help answer any further questions. Once our medical team receives your medical history form and medical records, our Patient Advocate will schedule an e-visit with Dr. Gonzalez to review your treatment options and determine whether or not you are a candidate to receive stem cell therapy.
Schedule Your Treatment
If your treatment is approved and you are interested in coming for stem cell therapy, your Patient Advocate will schedule your 4day appointment. Our Care Coordinator will then call and arrange your travel.
